Seller's declaration of exemption from full registration
In order to register as a Limited Telephone Solicitation, the Seller must declare themselves exempt from full registration by identifying the statute paragraph that best fits the Seller's business. Select the paragraphs that best fit your business by "checking" the paragraphs on the left. Once all identifying paragraphs relative to your business are checked, click the continue button.

The basic information page contains the information most prevalent to the registration. Fields with asterisks by them are considered optional and are not required to complete the registration. All required fields must have valid entries before clicking the continue button, otherwise an error message is produced. There are four main information areas:

Legal Business name and address - Enter the legal name of your business (even if it is a DBA) along with the address and telephone number for the principal place of business for the business. You may use two address lines, but please use city, State (select from list) and Zip code (use a - if using the + four digits, ex 85007-1784). Please include area codes in telephone numbers, for instance (602)542-1687x200
Fictitious names and "Doing Business As" names are acceptable as the registered name of the telephone solicitor. However, all names the business will be conducting business under must be registered with the Secretary of State. If you require additional lines for DBA names, see "Adding a DBA".
Service of Process Agent - this is the individual or organization that may receive legal process on behalf of the registering telephone solicitor. All registrants must have a service of process agent. In the case of limited telephone solicitors, the Office of the Secretary of State may receive service of process for the business. If you wish for the Office of the Secretary of State to do so, please fill in the Service of Process area with:

Filing Agent - this is the individual filing with the Office of the Secretary of State the Limited Telephone Solicitation Registration form.

Once all the required fields are completed, click the continue button.

Review and correction of basic information
The review information page provides the user with a chance to review and confirm the information being provided as a Limited Telephone Solictation Registration to the Office of the Secretary of State. The user may hit the "BACK" button within the browser if a mistake is found. The user may also add additional DBA names by clicking the "Add a DBA" button at the lower left of the screen.
For each DBA added, the registrant must provide the fictitious business name along with the office and address at which the name is registered.
Once the information is reviewed and all changes are complete, click the continue button.

Agreement of Declaration of an ON-LINE filing with the Office of the Secretary of State
The LTSA ON-LINE registration consititutes a filing with a state agency. Filing false or incorrect information with a state agency may result in a felony or misdemeanor, so please, PLEASE read ALL of the Declaration of an ON-LINE filing prior to hitting the AGREE button.
Once you have read and agree with the Declaration, click on the AGREE button to COMPLETE the filing process.
