ATTENTION – IMPORTANT NOTICE

Identification Requirements for Tradename, Trademark, Notary, Partnership, Athlete Agents, Charitable Organizations, Contracted Fundraisers, Telemarketers or Telephone Solicitors, Out of State Landlord Agents for Service and Professional Employer Organizations

A copy of identification must be provided with all applications indicating the name of the applicant as it appears on the form (please refer to section 41-1080, A.R.S. for exceptions.)

Pursuant to section 41-1080, the following forms of identification are acceptable:

1. An AZ driver license issued after 1996 or an AZ non operating identification license.
2. A driver license issued by a state that verifies lawful presence in the U.S.
3. A birth certificate or delayed birth certificate issued in any state, territory or possession of the U.S.
4. A U.S. certificate of birth abroad.
5. A U.S. Passport
6. A foreign passport with a U.S. Visa
7. An I-94 form with a photograph
8. A U.S. citizenship and immigration services employment authorization document or refugee travel document.
9. A U.S. certificate of naturalization.
10. A U.S. certificate of citizenship.
11. A tribal certificate of Indian blood.
12. A tribal or bureau of Indian affairs affidavit of birth.

All applications not including this information will not be processed.
